Document the X-Spam-Level check. Also add in a README file

which explains exactly how Telaen handles SPAM. Make it
clear that Telaen doesn't filter or "mark" messages as
SPAM, but rather is simply SPAM-aware. Most likely need more
wordsmithing on this.

Another big looking cleanup that is actually simple:
...
Remove the sid/tid/lid stuff from the URLs. There's no
need to pass those along there, but best to place
them in the user's session data. The URLs look much
nicer now and the URL params are only for stuff
that change from request to request...
